Here we present a case of a four year old boy with a violaceous non-blanching macular rash erupting on bilateral toes during the COVID-19 pandemic, later presenting with pustular drainage from his nailbed and MRI confirmed osteomyelitis of the digits affected by the rash. A workup to identify other sources of infection was negative. This patient required inpatient antibiotics and surgical intervention, though later made a full recovery.
